My teeth are so small. What can I do to make them look more normal?

All of my permanent teeth came in very small. My friends think they are still my baby teeth and I am becoming very insecure about my smile.  What can I do to my permanent teeth to make them look a more normal size?

Dear Tina,

We understand the concern about your permanent teeth and how it may make you feel. When our permanent teeth do not develop correctly it is called anodontia, most common in premature babies however can be a genetic disorder as well. There are also certain environmental factors that can influence teeth development. The good news is that there are some amazing dental techniques that can greatly improve your smile and bring back the confidence you are looking for.

Depending on the shape and alignment of your teeth you may be a candidate for porcelain veneers or porcelain crowns, both of these procedures are permanent, long lasting, and look very natural. Porcelain veneers cover just the front surface of the tooth, requiring very little reshaping of the tooth surface. Porcelain crowns require more shaping of the tooth surface and cover the entire tooth.

We recommend you schedule a consultation with a dentist that specializes in cosmetic dentistry and has a lot of training in the field of cosmetics. Choose wisely while searching for a dentist because cosmetic dentistry is a skillful art and takes much talent.

Which would be best – porcelain veneers or Invisalign?

For years I have dreamed of having a really beautiful smile. I recently came into some money and decided to get porcelain veneers on my upper teeth. There is nothing major wrong with my teeth. They are a little small, and pretty stained, but overall structurally OK. One tooth is crooked, but it is not so bad that I ever considered braces.

And that is kind of why I am confused. The dentist I am working with says I should do Invisalign braces before we place my veneers, and that is kind of confusing to me. I thought the porcelain veneers would take care of any crooked teeth. Aren’t they also referred to as “instant orthodontics”? My dentist says that after I have the Invisalign treatment, the prep required to place the porcelain veneers would be minimal, which certainly sounds like a bonus, but is it really worth the time and expense? A year of treatment, and thousands of dollars don’t seem like a good trade for a few millimeters of tooth that will be under a veneer anyway. He wants to place Lumineers, but I’ve been reading kind of negative things about them on the web.

Dear Cecelia,

Without seeing your case, it is impossible, of course to make definitive recommendations. It may be that you have a bite problem you are not aware of, or serious crowding issues. You really need to pursue this inquiry with your dentist, as it is quite unusual to recommend both procedures. Normally you would do porcelain veneers OR Invisalign, but not both together.

In this case, a second opinion is necessary. Go to a well recommended, experienced, artistic cosmetic dentist, and tell them what you want to have done. For the most accurate results you really should see a cosmetic dentist that also does Invisalign treatments. Don’t tell the second dentist about the recommendations of the first dentist until after he or she has offered an opinion. In this way you can get the truest assessment of the second dentist’s opinion on the best treatment options.

Just one crooked front tooth – what are my options?

I’ve been reading through all of your blog posts, and think you have a great site. I feel like I could really trust your advice, so I am going to ask what you think about my situation.

I have one crooked front tooth, and when I say crooked, I mean SERIOUSLY crooked. It has always been embarrassing to me, and now that I am getting ready to enter a new career, I want to get it fixed. The problem is that I keep getting different advice from different dentists! This makes me really distrustful. I don’t know who to believe, and was hoping you could help shed some light on the situation.

One dentist said I should go with conventional braces, and another said that I could try Invisalign. I was wondering about just pulling it and replacing it with a dental implant, but neither dentist would discuss that option. A third dentist said that I could consider a porcelain crown, but that it would be really challenging to match to my other teeth and that I would really want to talk to an expert cosmetic dentist about getting this work done. Does it really make that much of a difference? And why the three different answers? It makes me suspicious that this is all some big conspiracy.

Let us put your suspicions to rest. You received three different answers (all of them perfectly viable) because the three dentists you consulted all have different areas of expertise.

Conventional braces would certainly straighten your crooked tooth, but it could take up to two years to complete treatment. Invisalign treatment is usually shorter, and there are many advantages to that type of invisible braces. And finally, the dentist who advised you to talk to an expert cosmetic dentist was absolutely correct in stating that matching a porcelain crown or a porcelain veneer in the front of the mouth can be quite tricky. It is commendable that he or she recognizes this challenge and is giving you excellent advice.

Any one of the three solutions could work for you, so now it is time to decide which course of treatment would be best. It might be best to talk to an expert cosmetic dentist who also does Invisalign, as they could offer you the pros and cons of each solution.

Fluorosis stains on my kid’s teeth

I’m writing for some clarifying advice, I hope. I am pretty sure I will need to find a new dentist, but could use a more educated opinion.

My youngest daughter has pretty bad staining on her teeth, from fluorosis. Her bottom teeth in the front are really yellow (they are permanent teeth), and her top teeth are blotchy and in some cases almost jagged on the side (those are still baby teeth, though).

She wears braces right now, to try and fix issues related to a long thumb-sucking habit. We put braces on before her 12-year molars came in to try and give her some more confidence in her smile sooner. She is REALLY self-conscious about her teeth, but I think now the discolorations and staining bother her more than the overbite. She is very shy.

It is hard when your child suffers from confidence issues, and hopefully you can get connected with a skilled and experienced cosmetic dentist who understands the psychology behind appearance as well and the best techniques to enhance her smile.

If her staining is indeed severe, then the teeth must eventually be covered with porcelain veneers. Fluorosis is caused when the teeth are forming, by over-consumption of fluoride. This is sometimes caused by drinking water from public or private wells that has a naturally high level of fluoride over the course of several years when the teeth are developing. It can also be caused by a child repeatedly swallowing too much fluoridated toothpaste. Just as a note, your family dentist should reiterate each time you visit that a pea-sized amount of toothpaste is appropriate, and that you should make sure your child spits it out rather than swallowing.

It is important that you are confident your dentist knows the source of the staining before treatment. If your daughter’s bottom teeth are a uniform yellow (no blotchiness), it may not be from fluorosis, and a simple teeth whitening treatment may be sufficient. If the fluorosis staining is mild, it is possible that a skilled cosmetic dentist could cover the spots with dental bonding.

The key theme here is “skilled experienced cosmetic dentist”. Any dentist can say they do cosmetic work. But you want someone who can prove their skills with before and after pictures and evidence of accreditation and coursework. If you choose your dentist well, they will do a good job choosing the best treatment option for your daughter.
